Rather than ending support abruptly, this plan ensures participants know what to watch for, when to reach out, and how to respond if recertifications, reporting requests, income shifts, or household changes occur. By outlining current benefits and work status, identifying upcoming events, and creating a simple response protocol, the tool reinforces that reaching out during transitions is a strength—not a setback—and helps protect long-term stability.
How to Use This Tool: Complete this plan when a participant’s situation stabilizes, benefits shift, income increases, or sessions become less frequent. Document current benefits, work/income status, and priority stability areas. Review common triggers that should prompt outreach—such as renewal notices, income changes, or portal messages—and clarify preferred contact methods. Identify upcoming events within the next 30–90 days and outline immediate response steps if something changes. Create a simple “Stability Buffer” plan with a first step, backup option, and bridge support. Revisit and update this plan periodically to ensure transitions remain supported, predictable, and participant-led.
