How to Use This Tool: Begin by clarifying the participant’s current reality, including benefits in use and any stability concerns. Collaboratively define a meaningful goal tied to their values, then explore options and identify concrete next steps with target dates. Use the Milestone Tracker to monitor progress, document needed supports, and note potential benefit impacts. Complete the Top 3 Actions for the next 30 days to maintain forward momentum, and regularly update the Recertification Tracker to align goal progress with reporting requirements and income thresholds. Revisit this tool in ongoing sessions to adjust plans, celebrate milestones, and ensure that progress feels safe, paced, and participant-led.
